Why is this service needed?

Our society and our healthcare system spends the majority of its focus and resources on preventing death — which is fine when we are healthy.

But often times, conversations about the realities of declining health - and the inevitability of death - are delayed for far too long. Even when someone has a known life-limiting condition, discussions about palliative and hospice care are not introduced until there have already been months of confusion, frustration, and unnecessary suffering.

As a result, people are often unable to realistically evaluate and adjust their goals and decisions along the way. And they often reach the end of life without a clear picture of what to expect, what it will take to care for someone they love, or what help is available to them.

Walk Me Home offers a different path
through the transitions of life and death.

Walk Me Home offers people and families with serious or late-stage illness the opportunity to have authentic, helpful, and realistic conversations about the end of life.

Pioneered by a lifelong hospice nurse, this service provides support and guidance completely tailored to your family’s needs, so you can you stop fearing the future and stay present and connected in the time you have with the people you love.
